Appointments

Non-urgent advice: Online Submissions

You may be asked to verify your mobile number, if the is the case can you please go through the verification process, this will ensure your request is matched to your records which will make the process quicker for the triage team.

The online form you submit will ask for information in order to allow the GP to triage your request, and direct you to the most appropriate member of the team. Please add as much detail as possible, including attaching pictures where appropriate (eg for skin issues, avoiding pictures of any intimate areas). This enables us to deal your request more effectively and timely, and in the most appropriate way. If little or no information is added to a triage request, we cannot deal with your request effectively and cannot guarantee you an appointment which, if offered, may be up to 4 weeks away.  All information will be treated in the strictest confidence and is designed to help minimise the time you have to wait for your appointment.
